Below are listed the forms required for the LWML–Canada convention to be held in Kelowna, B.C. July 5-8, 2012.

NOTE: If you should fill out your general registration and tour forms and attach to an e-mail, it will require sending the credit card payment form by mail since we do not have secure Internet access. Cheques, of course, must be sent by mail.
(Please note that the Credit Card Payment Form does not apply to the Hotel Reservation Form…they will require your credit card information separately. Nor does it apply to the Mission Walk Donor Form because, if donors are to receive a tax receipt the cheque or cash must come from them, not the walker…except where the walker is the donor.)
